Brazilian Butt Lift (BBL) in Istanbul

A Brazilian butt lift (BBL) reshapes the buttocks using your own fat, harvested by liposuction and transferred to the buttock area. It has historically been associated with a higher risk of serious complications than many other cosmetic procedures, so technique, surgeon selection and hospital setting are critical. Results vary between individuals.

What is Brazilian Butt Lift (BBL)?

A Brazilian butt lift combines two stages: liposuction to harvest fat from areas such as the abdomen, flanks, back or thighs, and fat grafting to transfer purified fat to the buttocks. Because liposuction of the waist and lower back is part of the procedure, it can also change overall proportions, refining the waist-to-hip transition as well as adding buttock volume and projection.

Unlike buttock implants, a BBL uses your own tissue, and some of the transferred fat is naturally reabsorbed in the months after surgery. The amount that can be safely transferred depends on how much fat you have available and on your surgeon's judgement. Women who are very slim may not have enough donor fat, and alternatives such as buttock augmentation with implants may be discussed. Safety technique, specifically subcutaneous-only fat placement, is central to modern BBL practice.

Who may be suitable?

  • You have sufficient fat in donor areas such as the abdomen, flanks or thighs
  • Your weight has been stable and is within a range your surgeon considers safe
  • You are in good general health with no history of clotting disorders
  • You are a non-smoker or willing to stop for the advised period
  • You can follow sitting restrictions and garment instructions after surgery
  • You understand the specific safety risks of BBL and accept a conservative, safety-led plan

What can this procedure address?

  • Flat or low-volume buttocks
  • Lack of curve between waist and hips
  • Hip dips or indentations at the side of the buttocks
  • Mild buttock asymmetry
  • Excess fat on the flanks, lower back or abdomen that can be used as donor fat

Consultation & assessment

An online assessment lets you send photos confidentially from several angles, along with your medical history, weight and goals. The surgeon can give an initial view on whether you have enough donor fat and whether a BBL is appropriate. You should also use this stage to ask how the surgeon ensures subcutaneous-only fat placement and where the surgery will be performed.

Before surgery is confirmed, you will have an in-person examination in Istanbul. The surgeon assesses donor areas, buttock shape and skin quality, reviews blood tests and your thrombosis risk, and discusses realistic volume, sitting restrictions, garments and the travel plan home. If the surgeon advises against BBL or suggests a more conservative plan, that advice should be taken seriously.

How the procedure is performed

Under general anaesthesia, fat is harvested from donor areas using liposuction. It is then processed, by decanting, filtering or washing, to separate healthy fat cells from fluid and oil.

The fat is injected through small incisions using blunt cannulas. In line with current safety guidance, fat is placed only in the subcutaneous layer above the gluteal muscle, with the cannula kept parallel or angled away from the deeper structures. Some surgeons use ultrasound to confirm the correct plane during injection.

The incisions are closed and a compression garment is applied over the liposuction areas. Pressure on the buttocks is avoided after surgery to support fat survival.

Anaesthesia

BBL is performed under general anaesthesia in a hospital operating theatre with an anaesthesiologist present throughout. The anaesthesia team monitors you continuously and plans thrombosis prevention and post-operative observation, usually including an overnight stay.

Recovery timeline

  1. Days 1–3

You remain under observation, usually overnight. Expect soreness and bruising in donor areas. You will lie on your front or side and avoid direct pressure on the buttocks.

  2. Week 1

Walking is encouraged. Sitting is restricted and a BBL cushion may be advised for necessary sitting. Your surgeon reviews you.

  3. Weeks 2–3

Swelling begins to settle. Many women return to desk work with a cushion or standing desk. Your surgeon confirms fitness to fly.

  4. Week 6–8

Sitting restrictions are gradually relaxed and exercise may be reintroduced with approval.

  5. 3–6 months

Some transferred fat is reabsorbed and the buttock shape settles. Swelling in donor areas resolves.

  6. 12 months

The final result is usually established. Results vary between individuals.

What to expect during the first week

  • Avoid sitting or lying directly on your buttocks; sleep on your front or side
  • Use a BBL cushion if sitting is unavoidable, placed under the thighs as instructed
  • Wear your compression garment on donor areas as instructed
  • Walk regularly to reduce the risk of blood clots
  • Take anticoagulant medication exactly as prescribed
  • Seek urgent help immediately for breathlessness, chest pain or confusion

When can I fly?

Flying after a BBL needs careful planning because of sitting restrictions and thrombosis risk. Many surgeons advise staying around 10–14 days before a long-haul flight. Use a BBL cushion under the thighs to reduce pressure on the buttocks, consider an aisle seat so you can stand and walk frequently, wear compression stockings, stay well hydrated and follow anticoagulant advice. Some women choose a seat with extra space or a flat-bed seat where available.

When can I return to work?

Desk-based work is often possible after 2–3 weeks if you can avoid prolonged sitting, for example by using a cushion or standing desk. Physically demanding roles may require longer.

When can I exercise?

Walking is encouraged immediately. Light lower-body exercise may resume after around 4–6 weeks, with strenuous activity and squats typically delayed until 6–8 weeks or as your surgeon advises.

Scars

BBL uses small incisions for liposuction and fat injection, usually placed in natural creases or the bikini line. These typically fade over 6–12 months.

Results

BBL can add volume and improve the waist-to-hip contour, but some fat is reabsorbed, so the early result is not final. Consented before and after photographs can be reviewed during consultation. Results vary between individuals.

How long do results last?

Fat that survives after the first months behaves like normal body fat and can be long-lasting, but it will change with weight gain or loss. Some women choose a second fat transfer later rather than placing a larger volume in one session.

Safety first

BBL Safety: What You Should Know

Brazilian butt lift has historically been associated with a higher risk of fatal fat embolism than many other cosmetic procedures. The main danger arises when fat is injected into or beneath the gluteal muscle, where large veins lie. If these veins are injured, fat can enter the bloodstream and travel to the heart and lungs, causing a pulmonary fat embolism, which can be fatal within a very short time. This is not a theoretical concern; it is the reason BBL has been the subject of specific international safety guidance.

Current professional safety guidance, including recommendations from a multi-society task force involving ASERF, ASPS and ISAPS, emphasises that fat should be placed only in the subcutaneous layer, above the gluteal muscle fascia, and never into or beneath the muscle. Guidance also highlights the use of appropriately sized cannulas, keeping the cannula angled away from deeper structures, and continuous awareness of cannula position. Some practices now use intraoperative ultrasound to confirm that fat is being placed in the subcutaneous plane. You should ask your surgeon directly how they ensure subcutaneous-only injection.

Surgeon selection matters greatly. Choose a surgeon with specialist training in plastic, reconstructive and aesthetic surgery, verifiable registration and specific, substantial experience with gluteal fat grafting using subcutaneous technique. Patient selection matters too: not everyone is a suitable candidate, and your surgeon may advise against BBL or recommend a smaller volume or a different procedure based on your anatomy, health and the amount of fat available. A surgeon who declines to operate, or limits the plan, is acting in your interest.

BBL should be performed in an accredited hospital operating theatre, not an office or clinic room, with a qualified anaesthesia team present throughout. The hospital setting allows continuous monitoring, immediate access to emergency and intensive care resources, and appropriate overnight post-operative observation. Ask where your surgery will take place and who will administer and monitor your anaesthesia.

Venous thromboembolism (VTE) prevention is another essential part of BBL safety, because liposuction of several areas, longer operating times, restricted sitting and long-haul travel all increase clotting risk. Your team should assess your risk and use measures such as compression devices, early walking, hydration and anticoagulant medication where appropriate. Seek emergency care immediately for sudden breathlessness, chest pain, coughing blood, confusion, or calf pain and swelling, both in Istanbul and after you return home.
