Buttock Augmentation in Istanbul

Buttock augmentation with gluteal implants uses soft, solid silicone implants to add volume and projection to the buttocks. It may be considered for women who would like fuller buttocks but have too little donor fat for a Brazilian butt lift, and it carries specific risks that need careful discussion.

What is Buttock Augmentation?

Gluteal implants are made of solid, cohesive silicone designed to withstand sitting pressure, and are different from breast implants. They are inserted through an incision usually placed in the natal cleft (the crease between the buttocks) and positioned in a pocket within or beneath the gluteal muscle tissue rather than directly under the skin, to provide soft-tissue coverage.

Placement is typically intramuscular, within the gluteus maximus, or subfascial, beneath the fascia covering the muscle. Intramuscular placement offers more coverage and may reduce visibility of the implant edges, while subfascial placement can allow a larger pocket but with thinner coverage. Implants are sometimes combined with fat transfer or liposuction of the waist and lower back to improve overall proportion. Your surgeon will explain which approach suits your anatomy.

Who may be suitable?

  • Women in good general health with a stable weight
  • Those with limited donor fat for fat transfer
  • Non-smokers, or those willing to stop well before and after surgery
  • Women who understand and can comply with sitting and activity restrictions
  • Those with realistic expectations about size, shape and possible future revision
  • No active skin infection or wound-healing problems in the area

What can this procedure address?

  • Flat or small buttocks with limited natural volume
  • Lack of upper buttock projection
  • Buttock asymmetry in suitable cases
  • Volume loss after significant weight loss, sometimes combined with other procedures

Consultation & assessment

You begin with an online assessment, sending photographs taken in standard positions and your medical history confidentially. The surgeon can give a preliminary view on whether implants, fat transfer or a combination is more appropriate, and discuss implant size and placement in general terms.

In Istanbul, an in-person examination is carried out before surgery is confirmed. The surgeon assesses your muscle and soft-tissue thickness, skin quality and proportions, reviews your health and medications, and discusses specific risks such as infection, malposition and wound breakdown. Pre-operative tests and anaesthetic assessment follow.

How the procedure is performed

Under general anaesthesia, with you lying face down, the surgeon makes an incision, usually in the natal cleft, and creates a pocket within the muscle or beneath its fascia on each side. The implants are inserted using strict antiseptic technique to reduce the risk of infection.

The incision is closed in layers. Drains may be used. Compression garments are applied and you are monitored in hospital, usually for one or two nights. The procedure usually takes around 2–3 hours, longer if combined with liposuction or fat transfer.

Depending on the surgeon and procedure, treatments may be performed at leading private hospitals in Istanbul, including LIV Hospital Vadistanbul, Memorial Bahçelievler and selected Acıbadem hospitals.

Anaesthesia

Buttock augmentation is performed under general anaesthesia administered by a qualified anaesthetist. Because you are positioned face down, careful anaesthetic monitoring is important. Measures to reduce the risk of blood clots are used during and after surgery.

Recovery timeline

  1. Days 1–3

Pain, tightness and swelling are expected. You will lie on your front or side and begin gentle walking early to reduce clot risk.

  2. Week 1

Drains, if used, are removed. Sitting is restricted. Wound checks are important because of the incision location.

  3. Weeks 2–3

Discomfort eases. Short periods of supported sitting may be permitted as advised.

  4. Week 6

Many restrictions are relaxed. Light exercise may resume with surgeon approval.

  5. 3–6 months

Swelling settles and implants soften into position.

  6. 12 months

The final shape becomes clear; scars continue to mature.

What to expect during the first week

  • Avoid sitting directly on the buttocks unless your surgeon advises otherwise
  • Sleep on your front or side
  • Walk gently several times a day to reduce the risk of blood clots
  • Wear compression garments as instructed
  • Keep the incision clean and dry, and follow wound care advice closely
  • Report fever, increasing pain, redness or discharge promptly

When can I fly?

Most surgeons advise waiting around 10–14 days before a long flight, after a review confirming wounds are healing. Flying after surgery increases the risk of blood clots, so wear compression stockings, stay hydrated and walk during the flight. Sitting for long periods may be uncomfortable, and your surgeon may advise a cushion or seating arrangement.

When can I return to work?

Desk-based work is often possible after 2–4 weeks, but prolonged sitting may be restricted for longer. Physically demanding work usually requires 6 weeks or more.

When can I exercise?

Light walking is encouraged immediately. Strenuous exercise, particularly lower-body training and activities that stretch the buttocks, is usually avoided for 6–8 weeks or longer, as advised by your surgeon.

Scars

The scar is usually placed within the natal cleft, where it is largely concealed. This area is prone to moisture and friction, so careful wound care is essential. Scars fade over 12 months or more.

Results

Implants add more predictable volume than fat transfer, though the achievable size is limited by tissue coverage. Consented before and after photographs of similar cases are reviewed during consultation. Results vary between individuals.

How long do results last?

Gluteal implants are designed for long-term use but are not lifetime devices. Ageing, weight changes and implant-related problems may lead to revision or removal in the future.

Safety first

Gluteal Implant Safety

Gluteal implants are associated with a higher rate of wound and implant-related complications than many other aesthetic procedures, largely because of the incision location, sitting pressure and the demands placed on the tissues. Infection, wound breakdown, seroma and implant malposition are the main recognised concerns, and some may require further surgery or implant removal.

Reducing risk depends on careful patient selection, a surgeon experienced specifically in gluteal implant surgery, strict sterile technique in an accredited hospital operating theatre, and close adherence to aftercare instructions, including sitting restrictions and wound care. You should be told plainly what to do if a problem develops after you return home.

Quality & Safety

Potential risks and complications

All surgical procedures carry risks. Your surgeon will discuss the potential benefits, limitations and individual risks with you before you decide whether to proceed.

  • Infection, which may require antibiotics or implant removal
  • Wound breakdown or separation along the incision, particularly in the natal cleft
  • Seroma, a collection of fluid around the implant, which may need drainage
  • Implant malposition, rotation or displacement
  • Visible or palpable implant edges and asymmetry
  • Capsular contracture, hardening of scar tissue around the implant
  • Haematoma, bleeding requiring a return to theatre
  • Sciatic nerve irritation causing pain or altered sensation in the leg
  • Deep vein thrombosis or pulmonary embolism
  • Need for revision surgery or implant removal

Choosing a surgeon

  • Choose a plastic surgeon with specialist training, registered with the Turkish Medical Association, and verify membership of relevant societies such as the Turkish Society of Plastic, Reconstructive and Aesthetic Surgery
  • Ask how many gluteal implant procedures the surgeon performs and how they manage complications
  • Review consented before and after photographs of similar cases, remembering that results vary between individuals
  • Confirm surgery will take place in a private hospital where the surgeon holds operating privileges
  • Make sure there is a clear aftercare plan, including what happens if infection or wound problems occur after you return home
