Perineoplasty in Istanbul
Perineoplasty repairs and reshapes the perineum, the area between the vaginal opening and the anus. It is often considered after childbirth, when a tear or episiotomy has healed with painful scarring or left the vaginal opening wider or less supported. A private, confidential assessment with an experienced gynaecologist helps determine whether surgery is appropriate.

What is Perineoplasty?
The perineum supports the vaginal opening and pelvic floor. Vaginal birth can stretch or tear these tissues, and an episiotomy or tear may heal with scar tissue that is tight, painful or irregular. In other women, the perineal body becomes thin and the vaginal opening wider, sometimes causing a sense of laxity, air entering the vagina or discomfort.
Perineoplasty removes scar tissue or excess skin and rebuilds the perineal muscles to restore support and a more comfortable shape. It has a clearer functional rationale than some purely cosmetic procedures, particularly when it treats painful scarring. It is often combined with vaginoplasty. Normal anatomical variation is wide, and your surgeon will discuss realistic expectations.
Who may be suitable?
- Women with painful or tight scarring from a previous tear or episiotomy
- Those with a widened or poorly supported vaginal opening after childbirth
- Women who experience pain with intercourse related to perineal scar tissue
- Those who have completed their family, or understand that a later birth may affect the result
- Women in good health with no active infection
- Non-smokers, or those able to stop before and after surgery
What can this procedure address?
- Painful, tight or irregular perineal scars
- A widened or gaping vaginal opening
- Thinning of the perineal body after childbirth
- Discomfort during intercourse related to the vaginal entrance
- Excess skin tags at the vaginal opening
Consultation & assessment
Your journey usually starts with a confidential online assessment. You share your obstetric history, details of any tears or episiotomy and your symptoms, and you may send information privately through a secure channel. The surgeon can explain whether perineoplasty, physiotherapy or another approach may help.
Surgery is confirmed only after an in-person examination in Istanbul, where the surgeon assesses the perineum, scar tissue, pelvic floor and anal sphincter function. If symptoms such as bowel incontinence suggest a previous sphincter injury, specialist investigation may be recommended first. Your expectations and wellbeing are discussed openly.
How the procedure is performed
The surgeon removes a measured diamond or triangular area of skin and scar tissue at the back of the vaginal opening. The underlying perineal muscles are then brought together to rebuild the perineal body and support the vaginal entrance.
The skin and vaginal lining are closed with dissolvable stitches. The aim is a comfortable, supported opening, not excessive narrowing, which can cause pain during intercourse.
Perineoplasty is commonly combined with vaginoplasty when there is also laxity of the vaginal canal. Your surgeon will discuss whether combining procedures is advisable.
Anaesthesia
Perineoplasty is usually performed under general or spinal anaesthesia, or local anaesthesia with sedation for smaller repairs. An anaesthetist will assess you and explain the options. Long-acting local anaesthetic is often used during surgery to aid comfort.
Recovery timeline
Days 1–3
Swelling and soreness are expected, particularly when sitting. Rest, use prescribed pain relief and stool softeners, and walk gently.
Week 1
Discomfort gradually eases. A soft cushion helps with sitting. You attend a check-up before flying home.
Weeks 2–3
Most women return to desk-based work and normal activities. Stitches continue to dissolve.
Week 6
Many women are cleared for intercourse, tampons and more strenuous exercise after review.
3–6 months
Scar tissue softens and the final comfort and appearance become clear.
What to expect during the first week
- Rest and walk gently several times a day
- Use stool softeners if advised to avoid straining
- Sit on a soft cushion and avoid sitting for long periods
- Rinse gently after using the toilet and pat dry
- Avoid tampons, baths and swimming
- Report increasing pain, bleeding or fever promptly
When can I fly?
Most women can fly home around 7–10 days after perineoplasty, once cleared by their surgeon. Sitting can be uncomfortable, so a cushion helps. Walk the aisle regularly, stay hydrated and wear compression stockings if advised to reduce the risk of blood clots.
When can I return to work?
Many women return to desk-based work after 1–2 weeks. Roles involving prolonged sitting, standing or physical work may require longer. Your surgeon will advise.
When can I exercise?
Gentle walking is encouraged early. Avoid cycling, running, heavy lifting and swimming for around 6 weeks, or until your surgeon confirms healing.
When can I have sexual intercourse?
Intercourse is typically avoided for around 6 weeks while the perineum heals. Your surgeon will advise. Initial tenderness is common, and a water-based lubricant can help; persistent pain should be assessed.
Scars
Scars lie at the back of the vaginal opening and within the vagina. They usually heal well and become discreet, though firmness may persist for several months. Occasionally scar tissue can cause tightness that needs treatment.
Results
For many women, perineoplasty reduces discomfort from scarring and restores support to the vaginal opening. Results vary between individuals. Consented before and after information may be reviewed privately during consultation to explain realistic outcomes.

How long do results last?
Results are usually long-lasting, but further vaginal births can stretch or tear the repaired tissues. Discuss future pregnancy plans with your surgeon.
Potential risks and complications
All surgical procedures carry risks. Your surgeon will discuss the potential benefits, limitations and individual risks with you before you decide whether to proceed.

- Infection
- Wound breakdown, which can occur in this area because of pressure and moisture
- Pain during intercourse, including from over-narrowing
- Painful or tight scar tissue
- Altered sensation
- Injury to the rectum or anal sphincter, which is uncommon but serious
- Blood clots
- Need for revision surgery
Choosing a surgeon
- Choose a gynaecologist or surgeon with specialist training and experience in perineal and vaginal repair
- Confirm registration with the Turkish Medical Association and membership of relevant professional societies, and verify these credentials independently rather than relying on marketing material.
- Ask how they assess pelvic floor and sphincter function before surgery
- Ask to review consented before and after information privately
- Confirm hospital privileges and a clear written aftercare plan
